An community in Akron, OH tells how they developed a Accountable Care Community to address the health status of their community. They had obesity rates that were quite high and rates for chronic disease that mimicked what we’d see nationally. Occurrences of hypertension, cardiovascular disease and asthma all had significantly higher rates. And then behaviors such as smoking associated with those diseases were a little bit worse than what was reported nationally.

Asthma Awareness Summit

Asthma is a serious, costly, but controllable chronic disease. Asthma still remains a major problem in Fulton County. According to the American Lung Association, 24,657 children and nearly 55,000 adults suffer from asthma in Fulton County. In an effort to combat this problem, the Fulton County Department of Health and Wellness, Division of Health Promotion has helped launch Fulton Asthma Improvement and Reduction (FAIR) coalition in May 2012 to focus on the state of asthma in Fulton County. May 2014 is almost here.

Please save the date to join us in the informational session entitled ” Asthma Awareness: Continuing to Put the Pieces Together & The Missing Piece “. This is one of the several events in Fulton County in May aimed at improving policies and reducing the health disparities associated with asthma. We are excited to identify and bring together major stakeholders, community leaders, local hospitals, asthma practitioners, medical experts, public policy makers, and grassroots organizations to share their experience with the current state of asthma care and control and to create healthy environments and achieve asthma control throughout the Fulton County. The proposed session will provide an opportunity to share perspectives on some of the most challenging issues continuing to surround asthma and identify areas that would be best addressed through a unified, cooperative and a collaborative effort.

Purpose: The proposed summit continues to launch a focus on the chronic disease issues in Fulton County. This dialogue between academic, governmental, and interfaith community stakeholders will attempt to define opportunities to address the health issues and identify policies and practices that can effectively decrease the impact of chronic diseases in our community.

The Interfaith Coalition of Fulton summit will increase awareness of health among faith communities and provide a collective interfaith voice for effective and long term policy, systems and environmental (PSE) changes throughout Fulton County. The highlights of the summit will be health literacy workshop, chronic disease prevention presentation and intercultural experience.

What’s in it for you: After the Summit, it is anticipated that Interfaith Coalition will accomplish the following:

*Find common ground for reinforcing successful, collaborative interfaith community partnerships
*Examine community and interfaith partnership approaches and health promotion strategies
*Join as a member of the Interfaith Coalition of Fulton and become a part of the health movement

Feed Forward: Findings, presentations, workshops resulting from our interfaith summit will contribute directly to the enhancement and improve collaborations in the interfaith community. We are proud to be a part of this movement to promote awareness that will benefit our faith community.

As faith communities united for the physical, spiritual and mental well being of individuals, the Interfaith Coalition shall support and empower leaders of all ages in the integration of faith and health within their local communities through engagement, education and policy change.

Mission:
Members shall share best practices, be a resource to one another, sustain collaborations, and teach health literacy among their congregations, ultimately working to align faith and health for better outcomes.

Goals / Strategies:
1.Develop environmental scan/assessment for church communities targeting a demographic area
2. Position faith-based organizations as “healthy food champions” and implement healthy living initiative
3. Conduct community outreach activity that encourage healthy lifestyles in diverse communities
